Tier 2 - Intake Form

YOUR LOVED ONE

SECTION 1 - Before you Begin

A Quiet Pause

Before sharing anything, we want to gently confirm what this step involves.

Tier 2 is different from the Guided Voice Reflection you have already completed.

In this experience:

  • You are not asked to record your own voice

  • You may share as much or as little as you wish

  • You may stop or step away at any time

The information you choose to share will be used only to create a narrated audio reflection spoken about the loved one you are remembering.

Nothing is created without your consent.

REQUIRED: Consent boxes (all required)

SECTION 2 - About the Person You're Remembering

This section asks for a few basic details about the person you are remembering.

You may keep this simple. Short answers are enough.

You may skip anything you don't wish to share.

The following questions are optional, but we can advise that the more content you provide, the more engaging the narrated reflection we produce will be.

For example, yourself, children, parents, siblings, other family members, friends, work colleagues, people they admired from close and from far...

3.7 A Memory or Moment

If one memory or moment comes to mind, you may share it here.

This can be something small or ordinary. One or two sentences are enough.

3.8 How did your loved one impact others?

Impact doesn't have to be dramatic or far-reaching. Even quiet support, kindness, or just showing up can matter deeply.

3.9 What do you hope people remember most about your loved one?

This question invites you to share what you feel captures your loved one's lasting presence. This might be a quality, a way they treated others, or something they stood for.

Is there anything you feel you still carry from this relationship?

"A phrase, a habit, a way of seeing the world - or nothing at all."

Is there anything you would like the narrated reflection to acknowledge or simply express?"
